A student health form is to be completed yearly or when a change in health status/condition occurs.
A medication form completed by the student's physician and a parent signature is required for any medication to be given at school. This includes all over the counter medications such as ibuprofen, acetaminophen, cough drops, etc.

Dietary forms are completed by a student's physician and signed by a parent, indicating the medical need for a special diet to be provided at school . Examples include: supplements, food allergies, lactose intolerance, gluten sensitivity.

A dental form filled out by a student's dental provider is requested upon school entry, 2nd grade, 7th grade, and 12th grade.

Needed to grant permission for Monongalia County Schools to obtain and release information and/or records regarding a student from or to the school, department, agency, or person identified on the form.
This form is provided to parents as a tool to assess the risk for sudden cardiac death. This form is intended to bring awareness to risk factors your child may have and to give parents an indication when a student should be evaluated for identified risk factors by a physician.
